Pre-Placement Frequently Asked Questions

Will the Departments still give the medical history questionnaires to applicants?

No. We have changed the process so that the history forms will be provided to the applicant by the contractor at the time of the medical examination. Some classifications require more than one questionnaire, the mix of questionnaires required for a given classification are subject to change, and the questionnaires themselves are also subject to revision.

Do we have to use OHP's Pre-Placement Work Order form?

The contractor must receive a written request that contains all of the information in the Pre-Placement Work Order form. However, a department could use an alternate form if it provides the same information. For example, a department could put the required information in a table or list format when ordering multiple exams from the same contractor at the same time.

What if we need expedited clearance on an applicant?

We appreciate that situations can arise in which departments may not be able follow our recommendations for allowing sufficient time to process medicals. In these cases, please call our office and we will try to meet your departments needs.

Will OHP still send medical clearance cards to the departments?

Yes. However, we are working on a new computer system which will allow departments to access medical status information on-line.

What if a contractor cannot accommodate my request for a large number of exams?

While one contractor at any given time may not be able to accommodate a large request, we should have sufficient capacity throughout our network of contractors to meet requests of substantial sizes at all times. Please review the Table of Pre-Placement Contractors, and contact another contractor qualified to perform the examinations that you need. Also, please notify Dr. Goldberg (rgoldberg@ceo.lacounty.gov) so that he can determine if the contractor is meeting its contractual obligations.

Will we still receive a clearance form from the contractors for Basic exams?

Yes, and the form has been changed to include restrictions in some cases.

What if we become aware of bad service from a contractor?
