How to Order a Pre-Placement Examination

This procedure has changed! Exams are no longer ordered by components. Instead, they are ordered by item number. Also, the appropriate medical questionnaire (there is now more than one!) will be given to the applicant on the day of the examination by the contractor.

1) Ask the applicant if he/she has been examined within the past year for any other County job. If so, please contact the OHP, and inquire as to whether a new medical is needed. In some cases, our staff can clear an applicant for a position based on a recent examination for a similar position.

2) Use the information provided in Selecting a Contractor and the Table of Pre-Placement Contractors to find an appropriate contractor.

3) Contact the contractor selected by phone or e-mail, and request an appointment for the applicant. The contractor must be told the applicant's item number. Thereafter, you will need to send by fax or e-mail a Pre-Placement Work Order to the contractor. Alternatively, the Work Order may be delivered to the contractor by the applicant on the day of his/her appointment. The Work Order can then be used as an appointment slip for the applicant to give to the contractor. Our contractors are required to have written authorization prior to performing any services for the County.
           
4) Instruct the applicant to report for his/her medical exam on-time with a photo ID.
