How to Order an Employee Examination

This procedure has changed! Exams are no longer ordered by components. Instead, they are ordered by examination type (e.g., Asbestos, Hearing Conservation, etc.). Also, the appropriate medical questionnaire (there is now more than one!) will be given to the applicant on the day of the examination by the contractor.

1) Use the information provided in Selecting a Contractor and the Table of Employee Medical Contractors to find an appropriate contractor.

3) Contact the contractor by phone or e-mail, and request an appointment for the employee. The contractor must be told which medical screening examinations are needed. Thereafter, you will need to send, fax, or e-mail an Employee Work Order to the contractor. Our contractors are required to have written authorization prior to performing any services for the County.   
           
3) Instruct the employee to report for his/her medical exam on-time with a photo identification.
