  • The Most Commonly Required Document (MCRD) lists important information that you may need when applying for certain services. The MCRD will also tell you where you can get the important information if you do not have them already.

What questions are asked a lot on an application for services such as Medi-Cal, Food Stamps, or CalWORKs?

  • The Universal Face Sheet (UFS) has a lot of information that is needed by County departments and other agencies to decide if you can get certain services. To prepare for applying for services, you may want to print the UFS, fill it out ahead of time, and take it with you to various County departments. You may be asked to give other information not on the UFS, but if you have it in hand, you will have a lot of information that most County departments need to decide if you will be able to get certain services.
