Can I e-mail the nomination form?
A: Yes, you may e-mail nomination form to: nominations@ceo.lacounty.gov
Q: Should
I provide photographs and press clippings of my nominee?
A: No, verification material is not required in the committee's
selection process. All applications and forms will become the
property of the ERP.
Q:
Is it necessary for the nominee to be a current employee to be
considered
for a nomination in the current year?
A:Yes, the nominee should be a current employee. The committee
will evaluate every nomination form
and
will determine
the required
eligibility
of every individual..
Q: How
can I increase the chances of my nominee being considered for
an award?
A: ERP Committee will evaluate the key attributes of the nominee
or team based on the responses and examples you provide. The more
specific you are, the better.
Please consider each criteria for the award for which you are preparing
a nomination.
Q:
Can you nominate an individual or team involved in a project that
was developed more than a year ago?
A: Yes, you may nominate a project that was developed more than
a year ago, especially if you have good measurement data on how
well
the
project met expectations. The project must have been completed or
implemented by the submission deadline.

Ceremonies
Q:
What type of recognition will the winners receive?
A: Award winners will be announced at a CEO department-wide meeting,
held each quarter. They will be invited to meet with David Janssen
at a
CEO
reception prior to the department-wide meeting. They will also receive
a certificate and an engraved desktop award. The award winner's
name will be engraved
on a
CEO perpetual plaque displayed on the 7th Floor. All award winner's
names will be posted on the ERP website with individual consent.
Q:
Will the award winner receive a plaque or other type of physical
award?
A: Each winner in each category will get ONE award (currently an
engraved desktop award with their name and the category). If there
is
a shared award between a team, each team member will get ONE award
each. In addition, a certificate will be given to each individual
and a copy placed in their personnel folder.
Q:When
will nominees know if they have won?
A: Winners will be notified
as soon as the final recommendations from the ERP Committee
are approved by the CEO.
Submission
Q:
When should the nomination form be submitted to the ERP committee?
A: A nomination form needs to be submitted on or before the due
date indicated on the calendar. The ERP
committee will review the applications every quarter.

Q:
Is any of the information on the nomination form made public?
A: The only information made public is the name of the individual
and a brief summary of the accomplishments of the award winner.
Individuals
concerned
about privacy or content
may be asked
to be excluded from being posted on the ERP website or review the
content beforehand.
Q:
Will the ERP Committee contact the nominees at any time?
A: No. The ERP
Committee may contact the person submitting the nomination form
for more information if needed.
Q:
How is a submitted nomination form handled in regards to making
it
public information?
A: Nomination forms submitted are not made public. All award winner's
names will be posted on the ERP website with individual consent.
